We want to call your attention to the Model of Care (MOC) training callout at the bottom of this newsletter. This is an important required training and this prompt will be included at the end of each newsletter, as it will continue to be a requirement for all newly onboarded members of the interdisciplinary team.

The Centers for Medicare & Medicaid Services (CMS) requires all providers to complete CCA Senior Care Options (SCO) MOC training annually if you are either:

1. Contracted to see CCA SCO members, or

2. An out-of-network licensed independent practitioner (LIP) who routinely treats and/or provides services to CCA SCO members.

This training helps ensure providers and SCO plans can seamlessly coordinate the care for this population with significant needs. The following steps must be completed by all current SCO providers (all members of the interdisciplinary team).

Step 1: Complete training

Please download the 2024 Senior Care Options Provider Training by clicking the button below.

Step 2: Submit attestation

Once you’ve completed the entire training, please click the following button to fill out the attestation form.

Please continue to share this training reminder with your practice. If you have any questions, please contact [email protected]
